Incoming Students must follow the link below and complete the Application Form on the Student Health Portal.
How to Request Your Accommodation Letter
Returning students who are registered with Accessible Education Services must request an Accommodation Letter every semester.
To submit your accommodation request for the upcoming semester, please complete the following steps:
Step 1: Go to the Student Health Portal
Step 2: Select the Accommodations tab.
Step 3: Click on Term Request.
Step 4: Choose the upcoming semester.
Step 5: Select Request Accommodations at the bottom of the page.
You will then be able to choose which accommodations you would like to use for each of your classes this semester.
After completing your request, you will receive an Accommodation Letter for each course.
Please share each letter with your professors for signature and return the signed letters to AES at your convenience.
